How to question a suspected medical billing error

Gather the bill, EOB if insured, receipts, and notes before contacting the provider. Describe the exact line or amount you question and request a written explanation or correction.

Work through the issue

  1. Identify the disputed date, service, code, amount, and reason for your question.
  2. Contact the provider’s billing office; keep the date, name, and response.
  3. If insurance processing is involved, ask the insurer to explain the EOB and its appeal process.
  4. Follow the relevant appeal, complaint, or dispute instructions and deadlines for your situation; save copies.

Synthetic example

A fictional bill asks for $90 after an EOB shows a $40 patient balance for the same claim. The patient sends both documents to billing and asks for an itemized explanation of the $50 difference.

Limits
